    Male cat neuter:  $30  (There will be a $40 surcharge for cryptorchid males.)

    Rabies vaccine/adjuvanted 1-3yr (must sign disclaimer):  $10 

    Rabies vaccine/nonadjuvanted Merial PureVax^®:  $20

    Distemper vaccine (FVRCP):  $10

    Feline Leukemia/FIV/Heartworm test (for cats older than 6 mos):  $30

    Feline Leukemia only test (for kittens less than 6 mos. old):  $15

    Microchip Implantation (free registration):  $25

    Left ear tip for feral cats:  no charge

    Ear mite treatment:  $16  (will be done automatically if mites are found)

    Dose of Advantage or Frontline:  $12 (done automatically if fleas or ticks are found)

    Tapeworm treatment injection:  $20  (done automatically if worms are seen)

    Cardboard carrier:  $10  (required if more than 1 cat is presented in a single carrier)

    Nail Trim:  $5

    Donation to help fund our Red Cross clinics for low-income pet owners:  $______

In the interest of public health and in accordance with local and state laws, rabies vaccine is required unless proof of current rabies vaccination is shown.  Animals must be presented in individual carriers or traps and may not be left overnight under any circumstances.   Cash-only payment will be due in full upon pickup.  You will receive a phone call with the total amount due.  We do NOT accept any other forms of payment for the clinic.  By signing this form, you are agreeing to allow any additional necessary treatments as described above.

This is a low-cost spay/neuter clinic for feline population control purposes.  It is intended for cats that are feral, stray, or in the care of a rescue agency.  Although the clinic is open to any member of the public, individuals with owned pet cats are strongly encouraged to seek these services through their regular veterinarian. 

I understand the risks that are inherent with any anesthetic procedure.  I hereby release Animal Birth Control of South Jersey and Best Friends Veterinary Hospital, as well as its veterinarians, employees and affiliates, from all liability and waive my right to any and all claims for damages, including death, of the above animal.
